Your Name
Address
Phone Number (in case there is a question on your order)
Email address (optional)
Requested name exactly as you would like it to appear
(please also include location, date or number if applicable for your design)
The style number of the design

Specify each Item:

T-Shirt (short sleeve or long sleeve)
Sweatshirt (regular crewneck or hooded)
Kids, Adult or Ladies
Size
Quantity

If billing address and shipping address are different, we will need:

To whom we are sending the order
Shipping address (please indicate the name of the company if shipping to a business)
Phone Number (in case UPS has a delivery question)
Gift Message that you would like included (optional)

Discount on Merchandise:

For 10+ pieces, 10% discount

Shipping charge per address:

For 1-4 pieces, $5.95
For 5+ pieces, Free Standard Shipping
Shipping to U.S. and it's territories only.

Payment Options

Money Order - signed and made payable to InkPixi
Check - signed and made payable to InkPixi
Credit Card - MasterCard, Visa, Discover, and American Express. Include 16-digit card number, month and year of the expiration date and the name of the cardholder exactly as it appears on the card.
Sorry, no COD's
There will be a $30.00 Service Fee charged for all money orders and checks returned to us by the bank for any reason
Checks that you write for payment to our company may be converted into an electronic debit.
Payment in U.S. funds only.
